I have now beaten the game three time and i'm looking over my items and there's a slot missing do i get it when i get all the achievements?

OMFG I WAS GONNA GO TOO HEAVEN!!! MY LAST BOSS WAS INA RIGHT? NERF HER TENTACLES BECAUSE WHEN THE BRIDGE STOPPED SHE WOULD JUST THROW THEM OUT LIKE THERES NO TOMORROW NOW THE BRIDGE IS GONE AND I HAVE NOWHERE TO GO!!!
